If you have any members that increased their life insurance or elected Short Term Disability during open enrollment, the forms need to be mailed into MetLife by November 30, 2007.
There are 2 separate forms that have to be used.
For Short Term Disability you need to use the Disability Enrollment Form
And Life Insurance you need to use the Statement of Health form.
